Russia has cut the time it takes to register a disability from a month to as little as three days for amputees, and veterans of the special military operation now receive prosthetics and rehabilitation support automatically, without filing applications, Deputy Labor Minister Dmitry Ligomina said in an interview with Lenta.ru.

Ligomina said that before 2020 the standard for establishing a disability was 30 days. Today, he said, amputation cases are processed in three days, less serious cases take up to ten days, and the average is five to seven days.
He said the shift came from digitizing the process. In the civilian system, he said, a patient no longer needs to apply directly to a medical and social expertise bureau, known as MSE. Instead, a medical commission at the treating clinic assembles an electronic file once it becomes clear a patient's functions cannot be restored, and the patient chooses how to be assessed: in person, remotely by correspondence, or through a bureau visit to the hospital. For complex cases, he said, a specialist can join remotely through a telemedicine-style link so the patient does not have to travel.

For military personnel, Ligomina said, the Labor Ministry coordinated with military authorities so that each treatment facility is linked to the nearest MSE bureau. Military doctors already run their own medical commission for insurance payouts, he said, and now assemble a disability file in parallel, so the patient can recover in hospital without handling paperwork. He said MSE specialists set the disability group and design a rehabilitation programme, choosing devices suited to the specific amputation, and the completed documents are passed to military authorities and then to the serviceman.
Automatic certificates and payments
Ligomina said the entire expert process runs on a single digital platform operated by the Social Fund of Russia, with 1,700 primary bureaus working online through the cloud. Once an expert marks an assessment complete, he said, the fund is immediately notified that a citizen has a disability.
He said a proactive mechanism applies to special military operation participants: the fund automatically issues electronic certificates, without any application, for all technical rehabilitation devices set out in the programme, including wheelchairs, canes and orthoses. The card number to which certificates are linked is included in the case file from the start. As a result, he said, three days after an amputation a serviceman already has a monthly payment, a disability pension and a set of certificates on his card, and can order rehabilitation equipment through a marketplace for delivery to the hospital.

Ligomina said the Defense Ministry handles prosthetics itself, with separate financing, and the process begins in hospital once a limb is ready for a prosthesis. The Defense Ministry passes data to the Labor Ministry electronically, he said, so that once a serviceman is discharged, officials already know what equipment he has and whether it needs replacing.
Rehabilitation centres and satisfaction
The Social Fund of Russia runs 12 rehabilitation centres nationwide, Ligomina said, which reached about 19,000 people last year, including family members, with a target of 22,000 this year. He said a satisfaction survey conducted through the Gosuslugi state services portal found 98 percent of respondents satisfied with the quality of rehabilitation equipment, a figure he called evidence the system had become invisible but functional for the people it serves.

He said the fund also uses a "life situation" principle: as soon as it registers a person's status, such as veteran or disabled, the system automatically sends a notification to the person's Gosuslugi account listing every available form of support.
Matching prosthetics to lifestyle
Ligomina said rehabilitation programmes are built around two factors: the medical nature of the injury and the person's profession, family situation and lifestyle. An active person who wants to run, swim or cycle typically gets an electronically controlled prosthesis, he said, while an older person with a quieter lifestyle may need only hydraulics for daily tasks.

He said prosthetists use an internationally recognised four-level activity model to determine components such as knee modules, feet and sockets, and that patients can progress to more complex devices, such as multi-grip hands, as their skills develop, or move to simpler devices as they age. Location makes no difference, he said: a resident of a remote village and a Muscovite with the same medical need receive the same device on the same timeline, since the process is determined by medical and social indicators rather than by the ability to pay extra for an upgrade.

He said some veterans of the Great Patriotic War still use decades-old leather prosthetics and decline silicone liners, and that in some regions patients still request wooden stump sockets made of aspen, a design still produced in Germany, because that is what they find comfortable.
Certificates versus state contracts
More than 95 percent of people choose electronic certificates over state-procured contracts for prosthetics, Ligomina said, because certificates let them order from marketplaces, choose brand, colour and features, and return items that do not suit them. He said 72 percent of spending on technical rehabilitation devices now goes through certificates, more than 140 billion rubles a year, with additional out-of-pocket payments by users amounting to less than 5 percent of the total.

Consumables such as catheters and absorbent products are often supplied through state contracts instead, he said, particularly for people in remote areas without specialist shops, since the fund can deliver a quarter's supply directly to their village.
Quality control and replacements
Ligomina said Russia's prosthetics standard requires a multidisciplinary team, including a traumatologist, technician, prosthetics engineer, physical therapy specialist, rehabilitation specialist and psychologist, to reduce the risk that a device does not fit.

He said broken modules are replaced under warranty, and that Russia's system pre-emptively allows for up to three socket replacements after initial prosthetic fitting because stump tissue continues to change shape. Some sockets are now built with adjustable volume so minor changes go unnoticed, he said, and none of this requires a new trip to the MSE bureau.

He said cases of substandard service are investigated when reported, but that the certificate system has introduced market discipline: users can refuse a company and demand their certificate be reinstated if a prosthesis fails to meet the required functionality, and companies face fines under the state procurement law, Federal Law No. 44, for every day of delay or breach of terms.

Training more prosthetists
Ligomina said prosthetists were previously trained only in Korolyov, in the Moscow region, and in St. Petersburg, but five educational institutions now run training programmes, with two more seeking licences, developed jointly with the Education Ministry under a new professional standard. He said more than 400 prosthetists are currently in training, including at a new college in Novokuznetsk, in the Kemerovo region, which also trains disabled students as technicians.

Education and jobs for veterans
Ligomina said veterans can receive a fully funded second higher education or vocational qualification, or take a shorter course through the employment service in one of 213 in-demand professions under the national "Cadres" project, 27 of which are reserved exclusively for special military operation veterans. Four federal operators design the programmes around employer demand, he said, and applicants can sign up through the "Work of Russia" portal.
He said the Labor Code guarantees veterans the right to return to their former job, and employers who cannot restore a veteran's old role must offer another position. Employers can also receive a subsidy of up to 200,000 rubles to create or adapt a workplace, along with wage subsidies of three times the minimum wage, rising to six times for hires with disabilities, plus compensation for relocation costs.

He said skills from military service often transfer directly to civilian jobs, citing drone operators moving into agriculture and tank engineers who can service trucks and excavators.
Domestic prosthetics industry
Ligomina said Russia's push to substitute imported components predates the special military operation, dating to a 2011 strategy with the Industry and Trade Ministry, and that Russian firms now produce electronic bionic hands and knees after years of writing software and algorithms from scratch to avoid patented Western designs.

According to a 2024 Industry Ministry strategy he cited, domestic products account for 51 percent of the market. He named wheelchair manufacturers Katarzhina, Tolyatti and Fakir as producing carbon-fibre chairs used by Paralympic athletes, and the Istok Audio plant in Fryazino as supplying a full line of domestically made hearing aids. Imported devices remain available and certificates can be used to buy them, he said, though patients then cover any difference in cost themselves.

Contrast with Western systems
Ligomina argued that Western insurance-based rehabilitation systems tend to restore a patient only to their previous occupation, whereas Russia's approach lets people change their lives entirely after injury. He described a joint programme with the Federal Medical-Biological Agency in which specialists in adaptive sport, retraining and driving instruction worked with patients in hospital, saying veterans have told him that seeing those possibilities gave them a reason to commit to their medical rehabilitation.
