To become a naturalized citizen of the United States, a person must pass the naturalization or citizenship test, which is taken during the naturalization interview.
During that naturalization interview you will have to answer questions about your form N-400, naturalization application and your background. You will also take an English and integration test, unless you qualify for an exemption or exemption.

Exceptions related to the English language requirement

An applicant seeking an exception to the English or citizenship requirements or both must submit a Form N-648 as an attachment to the Application for Naturalization (Form N-400).
Medical disability exceptions for the English and integration test
The person may qualify for an exception to the naturalization requirements of the English and Civic Integration Test if he or she cannot meet these requirements because of a physical, developmental, or mental disorder.
To request that exception, you must submit Form N-648, Medical Certification for Disability Exceptions.
This form must be completed by a licensed physician, certified osteopathic physician, or certified clinical psychologist.
What happens if I don’t pass the US citizenship test?
They mention on the USCIS site that they will give you two chances to pass the exam for the English or citizenship portion, or to answer questions regarding your application for naturalization, Form N-400.
If you do not satisfactorily answer any questions on these exams during your initial interview, you will be retested on the same portion of the exam that you failed (English or Social Studies) between 60 and 90 days after the date of your initial interview.
