Vaccination of children between 3 and 4 years old started nationwide on February 21. Parents and legal representatives must submit an informed consent so that children can access the vaccine. Find out what steps you must follow to perform this procedure.
The Ministry of Public Health (MSP) has issued a two-page document with information on the vaccine to be administered to children and on possible adverse reactions. This document is called “informed consent” and must be signed by the father, mother or, if applicable, the legal representative of the child.

On the second page you will find the form that you must fill out with your data (names, surnames, identity card number or passport)as well as the data of your daughter or son.
With this consent, the document clarifies, you indicate that you have understood the vaccination process, as well as the possible adverse reactions of the vaccine, and that you freely and voluntarily agree to the vaccination of your client.
