To obtain the so-called 500 plus for the disabled, it is necessary to submit an application. This can be done on behalf of the applicant by a guardian or proxy. Such an application can be submitted both at a ZUS branch and sent by post or electronically via the Electronic Services Platform. It is necessary to attach the required documents to the application, i.e. a certificate of total incapacity for independent existence, if ZUS does not have them.
