• income not exceeding PLN 1,727.08 gross
• a certificate of inability to exist independently

The application can be submitted by yourself or through a trusted person, or at the nearest office of the Social Insurance Institution.

In addition to the application, the senior must provide appropriate medical documentation that proves his health condition. Only then will the money be awarded. The documents include:

• medical records
• all other documents that may affect the examination of the application, e.g. documentation from a rehabilitation clinic or a health certificate (issued by a doctor no earlier than a month before submitting the application).
