PLN 4,000 after the death of a loved one. Who is entitled to the funeral grant?
of 17 December 1998 on old-age and disability pensions from the Social Insurance Fund strictly regulates which persons may receive a funeral allowance. These are:
- family members (spouse, stepfather and stepmother, own children, children of the spouse, adopted children, children raised as part of a foster family, other children adopted for upbringing and maintenance before they reached the age of majority, siblings, grandparents, grandchildren, and persons over whom established legal guardianship);
- employers;
- social welfare Home;
- commune and poviat;
- a legal person of a church or religious association;
- a stranger.
Family members will receive 4,000. PLN regardless of the amount of the funeral costs incurred, while other persons or institutions may count on the benefit up to the amount of the costs incurred, but not more than PLN 4,000. zloty. Claimants claiming a funeral grant must be able to prove that they incurred the costs of the funeral.
Funeral Grant. What documents should be submitted to ZUS?
Persons applying for the payment of a funeral allowance should submit the following documents to ZUS:
- application for payment of the funeral grant on the form;
- death certificate, birth certificate of a stillborn child or a complete copy of the child’s birth certificate with the additional note that the child was stillborn (a death certificate can be issued abroad, but it should not be temporary);
- originals of bills showing the funeral expenses, or if they were previously submitted to the bank, copies of these bills certified by the bank as true copies;
- documents confirming kinship or affinity with the deceased (abbreviated copies of civil status records);
- certificate of the contribution payer on being subject to old-age and disability pension insurance.
Documents must be submitted within 12 months of the death of the person whose funeral expenses were paid. An application submitted after this deadline will not be considered positively, unless the body of the deceased was found late or there were reasons beyond the control of the person who incurred the funeral costs. In this case, the application must be submitted within 12 months from the date of the funeral, as well as confirmation of the reason for the delay (official documents, certificate from the police or prosecutor’s office).
